When reopening or refreshing chats there should be reliable restoration of conversation state and no duplicate messages. Specifically, reloading (e.g., Ctrl/Cmd+R) should not create duplicate entries, conversations should restore properly when reopened, and new messages must be routed to the correct conversation so content never appears in the wrong chat.
